Mercy Home Health and Hospice of Mercy
Hospice of Mercy and Mercy Home Health have provided compassionate care in the community for over 30 years. Our team of nurses, aides, social workers, chaplains and volunteers will spend time with you, your physician and your family to create a customized care plan that meets your goals and needs. Mercy Home Health and Hospice of Mercy both partner with Centura Health at Home and Mercy Regional Medical Center to bring a continuum of expert, innovative, compassionate care to people of all ages, in all the places they call home.
We are dedicated to providing the best care possible, so our staff is fully trained and screened before they ever reach your door. And we're JCAHO accredited, which means we've demonstrated our ability to provide quality care. Whether you receive care in your own home, hospital or other residence, we'll make sure you experience the comforting feeling of home, because home is a state of mind as much as a place.
We are proud to serve patients in La Plata, Archuleta and parts of Montezuma counties. To contact us directly, please call 970.382.2000. For physician orders, you may reach us at 877.546.8253.
