What Medicare Requires For Us To Serve You
You are eligible for HOME CARE services if:
- Your doctor decides that you need medical care in your home, and makes a plan for your care at home.
- You need at least one of the following: intermittent (and not full-time) skilled nursing care, or physical therapy or speech language pathology services.
- You are homebound. (This means that you are normally unable to leave home. Being homebound means that leaving home is a major effort.)
- When you leave home, it must be infrequent, for a short time or to receive medical care.
- The home health agency caring for you must be approved by the Medicare program.
You are eligible for HOSPICE CARE if:
- You are eligible for Medicare Part A (hospital insurance).
- Your doctor and the hospice medical director certify that you are terminally ill and probably have less than six month to live.
- You sign a statement choosing hospice care instead of routine covered Medicare benefits for your terminal illness.
- You receive care from a Medicare-approved hospice program.
