  • A. Home Telehealth allows a nurse to monitor you daily in the comfort of your home with a small Telehealth machine. The Telehealth machine asks you questions about your symptoms and takes your blood pressure, pulse, oxygen saturation, and weight.
    A. The Telehealth unit is set up in your home through an electrical outlet and a phone jack. If you do not have a land line phone, the unit can be made wireless by linking to the nearest cellular tower (with no charge to you). The machine can be programmed by the monitoring nurse to ask you specific questions and take your vital signs. The data it collects gets sent to the monitoring nurse within minutes and the nurse will review the data and give you a call if needed.  Monitoring occurs during home care business hours, but you will still be able to call a nurse 24/7 with questions or concerns. 
    A. Any home care patient with a chronic illness such as Congestive Heart Failure (CHF), Chronic Obstructive Pulmonary Disease (COPD), asthma, High Blood Pressure, Diabetes, and more may be eligible. You must also be able to use the equipment or have someone in the home that can help you use it. The machine can be set for English or Spanish.
    A. By monitoring you more closely, the nurse can detect early on whether your chronic illness may be getting worse. Through early detection, the nurse will call your physician and adjust medications as needed to stop the progression of your illness. Because of this program, we may be able to keep you from going back to the hospital or emergency room. When the nurse calls you on the phone, she can help you understand your illness, your medications, diet, and exercise, all in an effort to keep you in a stable condition and remain in your home.
    A. There is no cost to the patient. If your home care nurse feels Telehealth would benefit you, we will work with your insurance company to cover the service. Telehealth is a benefit we provide to enhance the quality of life for our patients.
